About Tianeptine Sodium Salt: What Do You Know About It?
Tianeptine sodium salt is an analytical reference standard classified among opioid-related substances. Tianeptine has been associated with abuse, overdose, and fatal incidents in some cases. Preparations containing tianeptine have been used in the treatment of depression.
Tianeptine sodium salt is primarily intended for analytical and forensic research applications and may also be used as a general scientific research tool.
Applications and Pharmacological Characteristics of Tianeptine Sodium Salt
Tianeptine sodium is a tricyclic antidepressant with a mechanism of action that differs from traditional tricyclic antidepressants. It enhances the reuptake of 5-HT (serotonin) in the synaptic cleft, which may improve serotonergic neurotransmission. It shows no affinity for cholinergic or adrenergic receptors.
Tianeptine sodium is rapidly and completely absorbed through the gastrointestinal tract. It distributes quickly within the body and exhibits a plasma protein binding rate of approximately 94%. The compound is fully metabolized in the liver through β-oxidation and N-demethylation pathways. Its terminal elimination half-life (T1/2) is relatively short, around 2.5 hours. Only a small proportion of the unchanged drug (approximately 8%) is excreted via the kidneys, while most metabolites are eliminated through urine. In patients with renal insufficiency, the elimination half-life may be prolonged by approximately one hour.
Concurrent use with monoamine oxidase inhibitors (MAOIs) may lead to serious adverse reactions, including cardiovascular complications, paroxysmal hypertension, hyperthermia, convulsions, and potentially death. MAOIs should be discontinued for at least two weeks before initiating this product, while MAOI therapy may begin 24 hours after discontinuing tianeptine sodium.
Salicylates may reduce the plasma protein binding rate of this product; therefore, dosage adjustment may be necessary during combined administration.
Precautions for Using Tianeptine Sodium Salt
Use cautiously in patients with cardiovascular disorders, gastrointestinal diseases, or severe renal insufficiency.
Patients with depression and suicidal tendencies should be carefully monitored during treatment, especially during the early stages.
If general anesthesia is required, the anesthesiologist should be informed that the patient is taking this medication. It is generally recommended to discontinue use 24–48 hours before surgery. In emergency surgical situations, withdrawal may not be necessary, but careful perioperative monitoring should be performed.
As with other psychiatric medications, treatment discontinuation should involve gradual dose reduction over a period of 7–14 days.
Some individuals may experience reduced alertness or drowsiness; therefore, drivers and machine operators should exercise caution while using this medication.
Before initiating treatment with this product, monoamine oxidase inhibitors should be stopped for at least two weeks. Patients switching from tianeptine sodium to MAOI therapy generally only need a 24-hour discontinuation period before starting MAOIs.
